The Erase User box - which moves users to a banned user group, and
undoes everything they've done on the site including deleting comments,
undoing moderation and hiding stories - has been around for a LONG time
but some people find it difficult to deploy in that it requires creation
of an OP, so to make that easier, I've attached a set of sql statements
which will fully deploy the erase_user mechanism provided a more or less
default user_info_page
